Mission Statement
The mission of the Re-entry Housing Program is to empower individuals transitioning from
incarceration by providing a safe, stable housing, and a comprehensive support service. We are
committed to fostering successful reintegration, by reducing homelessness and recidivism, and
promoting self-sufficiency through collaboration with community partners with a strong focus on
dignity, respect, and opportunity for all participants.
Program Statement: Re-entry for Incarceration Housing Program
The Re-entry for Incarceration Housing Program is designed to support individuals transitioning
from incarceration back into the community by providing safe, stable, and affordable housing
options. The program aims to reduce recidivism, promote successful reintegration, and enhance
public safety through comprehensive housing assistance and supportive services.
Program Scope:
This program serves eligible individuals who are exiting correctional facilities and are at risk of
homelessness or unstable housing. Services are available regardless of race, gender, religion, or
background, and are tailored to meeting the unique needs of each participant.
Program Objectives:
1. Provide Transitional and Permanent Housing – by ensuring access to secure housing upon
release, including emergency shelter, transitional housing, and pathways to permanent residency.
2. Support Reintegration - by offering case management, life skills training, employment
assistance, and connections to community resources to facilitate successful re-entry.
3. Promote Self-Sufficiency – by empowering participants to achieve independence through
individualized service plans and ongoing support.
4. Reduce Recidivism - by addressing barriers to housing and stability that may contribute to
repeat incarceration and fostering long-term positive outcomes.
5. Collaborate with Community Partners – by working closely with local agencies, landlords,
employers, and service providers to maximize resources and opportunities for participants.
Program Implementation:
The program will be administered by trained staff who conduct assessments, develop
personalized housing plans, and coordinate supportive services. Regular monitoring and
evaluation will ensure program effectiveness and continuous improvement.
Services Offered by the Re-entry Program
1. Transitional and Permanent Housing Assistance
- Placement in temporary or transitional housing upon release
- Support in securing permanent, affordable housing
- Assistance with rental applications, deposits, and landlord mediation
2. Case Management
- Individualized needs assessment and service planning
- Ongoing support and progress monitoring
- Advocacy and coordination with other agencies
3. Employment and Job Readiness Services
- Resume building and job search assistance

- Connections to local employers and job fairs
- Vocational training and educational opportunities
4. Life Skills Training
- Financial literacy and budgeting workshops

5. Mental Health and Substance Use Support
- Referrals to counseling and therapy services
- Access to substance use treatment programs
- Peer support groups and recovery resources
6. Legal Assistance
- Help with obtaining identification documents (ID, Social Security card, etc.)
- Guidance on expungement, parole/probation compliance, and legal rights
- Referrals to legal aid organizations
7. Health and Wellness Services
- Assistance with enrolling in health insurance
- Referrals to primary care, dental, and specialty providers
- Health education and wellness workshops
8. Family Reunification and Parenting Support
- Counseling for family relationships and reunification
- Parenting classes and child support guidance
- Mediation services
9. Transportation Assistance
- Bus passes or transportation vouchers
- Coordination of rides to appointments, interviews, and housing viewings
